Forms & Contract Templates

Please read the descriptions of each form and contract template to determine which one fits your unique situation. Then email intellectualproperty@kennesaw.edu if you have additional questions.

  • The purpose of this Intellectual Property Disclosure Form is to record intellectual property that was created and the circumstances under which it was created.  It provides the basis for a determination of patentability, for drafting a patent application, and/or for registering a copyright or trademark.  This is a legally important document, which should be prepared carefully.

An Intellectual Property Disclosure Form should be completed when something new and useful has been conceived or developed, or when unusual, unexpected, or nonobvious research results have been achieved.  Where appropriate, the University endeavors to patent intellectual property created by its faculty, staff, and/or students through its Research and Service Foundation (KSURSF) and license such intellectual property to industry for further development and commercialization.  Royalties derived from any such license will be shared with the inventor(s) in accordance with the University’s Intellectual Property Policy

  • The purpose of this contract template is to provide a standardized option for sponsored capstone projects across academic units. This template is meant to address most sponsored capstone arrangements and avoid a lengthy contract preparation and review process but may not be appropriate for all instances. Capstone course instructors are encouraged to use this contract template, paying attention to its preamble that underpins the legal construct of this contract; however, if this contract template is inadequate the capstone instructor may work with the Office of Intellectual Property Development (OIPD) and the Division of Legal Affairs to establish a more appropriate contract with the project sponsor. The form should be filled out with the appropriate dollar amounts and other required information, and then signed by all parties before emailing an electronic copy to OIPD for KSURSF’s signature and submission into KSU’s Contract Management System.

  • The purpose of this contract template is to provide a standardized option for faculty that engage in consulting or “service for hire” projects as part of their employment at KSU. This is distinct from any outside consulting that a faculty member may engage in, and only address agreements routed through KSU (typically the faculty member’s College) rather than through KSURSF. This template is meant to address most consulting arrangements and avoid a lengthy contract preparation and review process but may not be appropriate for all instances. Faculty are encouraged to use this template; however, if this contract template is inadequate then the faculty member may work with the Office of Intellectual Property Development and the Division of Legal Affairs to establish a more appropriate contract with the sponsor. The form should be filled out with the appropriate dollar amounts and other required information, and then signed by all parties before emailing an electronic copy to OIPD for KSURSF’s signature and submission into KSU’s Contract Management System.
